The following items must be checked and adhered to:		Aware of wind and weather conditions that may cause the controlled burn to spread. Controlled burn will be 50 feet or greater from any structures or overhead wires/trees. Will attend the fire at all times until fully extinguished. Will begin burning at or after sunrise. Will have fire completely extinguished one (1) hour prior to sunset. Will call 9-1-1 if fire gets out of control.
Acknowledgement:		I acknowledge that I have read and understand the applicable burning ordinance for the municipality in which the controlled burn will occur. I will not burn materials prohibited by the applicable ordinance and/or by the regulations of the Department of Environmental Protection. Furthermore, I understand that any violation of the applicable ordinance and/or regulations could result in a citation answerable in Magisterial District Court in accordance with the policies and ordinances of the municipality having jurisdiction.
